Thank you for signing up for coaching. Please fill in this form so you can understand yourself a little better before your coaching begins, and help me get a better understanding of you too. We’ll go through this information in detail during your session(s).

Our coaching will focus on you as a whole person, and what wellbeing means to you.

 

Whilst you fill in this information, create yourself a cosy sanctuary, take a moment to sit, find a quiet spot where you won’t be distracted or disturbed.

 

Take a deep breath, let it go, repeat this three times.

 

There are no right or wrong answers. You can write as much as you like, in fact I encourage you to. Please be completely honest in your answers. It’s about finding what wellbeing means to you and finding an indication.

At some points in life we have transitionary seasons when things happen like becoming a parent, when our children fly the nest, when we change career, when we retire, when we experience burnout, bereavement, grief, traumatic experiences, physical or mental illness, life's challenges – and any other reasons we go through a change in circumstances or identity when we might need to do things like adapt, reflect, heal and/or bring a different energy or tempo into our life
